Protection of personal data

We process personal data in accordance with the requirements of Regulation (EU) No. 2016/679 of the European Parliament and of the Council on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation) or also GDPR).

MUDr. Petr Tomaštík is personal data controller and he processes the data mainly for the purpose of

  • provision of health care,
  • mandatory reporting to ISIN (infectious disease information system)
  • billing of health care to health insurance companies.

Your personal data is collected and further processed mainly on the basis of a legal obligation arising from Act No. 372/2011 Coll., On health services and the conditions for their provision (the Health Services Act), within which your identification, address, contact information is collected. Data, information about your health condition and the course of provided health services, anamnestic data and other decisive data necessary for the provision of a specific health service (according to the nature of the service provided). In some cases, your personal data may be collected and further processed on the basis of your consent, which always expresses a free, specific, informed and unambiguous expression of your will to process your personal data for a given purpose and always includes information about the possibility of withdrawal of the consent at any time.

When processing your personal data, there is no automated decision-making, on the basis of which actions or decisions would be made, the content of which would be an interference with your rights or legitimate interests. Your personal data is processed only for the necessary time, which is individual for each purpose of processing. After the expiration of this period, personal data are destroyed or are further stored for the period specified in the valid File and Disposal Plan, issued in accordance with Act No. 499/2004 Coll., On archiving and the file service.

In connection with the processing of your personal data, you have the right to access, correct or delete your personal data, or restrict processing, object to their processing, or exercise the right to data portability and other rights under the General Data Protection Regulation. Apply your rights and requirements to the Data Protection Officer. Your requirements will always be properly assessed and settled in accordance with the relevant provisions of the General Regulation. In the event that you do not agree with the settlement of your requests and applications, you have the right to file a complaint with the Office for Personal Data Protection.
